For copies of an Employee's medical records, an email must be sent to Risk Management authorizing the release of said records. Please include your name, maiden name (if applicable), date of birth and last four (4) numbers of your social security number. Note: Do not use this form to report employee injuries. If an injury occurs because of a motor vehicle accident, the employee must also fill out an employee report of injury.
